I don't know for a fact who wanted it changed, but "Puerto Rico/you ugly island/island of tropic diseases" is Anita's counter to Rosalia's first line in the first verse of the song as sung on stage: "Puerto Rico/you lovely island/island of tropical breezes." Without Rosalia starting the song with that line, the response loses much of its bite.

It's possible someone thought the line about tropic diseases was too risqué, but since the line about a social disease was left in "Gee Office Krupke" I doubt it.

Funny that Rita was OK with singing "let it sink back in the ocean," which isn't particularly kinder.
